“I understand that participation in dance classes, rehearsals, performances and related activities involves inherent risks, including the risk of personal injury, illness, property damage or loss. I voluntarily accept these risks.

I confirm that I, or my child, am medically fit to participate in dance activities and will notify Balance Dance School of any medical condition, injury or change in circumstances that may affect safe participation.

To the fullest extent permitted by law, I release and hold harmless Balance Dance School, its Principal, teachers, employees, contractors and volunteers from liability for any loss, damage or injury arising from participation in dance activities, except where liability cannot lawfully be excluded under the Australian Consumer Law or other applicable legislation.

I understand that while reasonable care is taken to provide a safe environment, participation in dance activities is at my own risk.”
